2. Visiting our website

We use Google’s visitor and ad measurement tools (Google Analytics and Google Ads) only with your consent (see section 3). When you visit the website, the server automatically records technical data (IP address, time of access, page visited, browser type) in server logs. We do this to keep the website running securely, based on our legitimate interest (Art. 6(1)(f) GDPR). The logs are processed on our behalf by our hosting provider, WebSupport s.r.o., as a processor, and are kept only for as long as necessary.

This privacy policy page loads the Bootstrap stylesheet from the public server stackpath.bootstrapcdn.com. When it loads, this server receives your IP address; it does not set cookies.

3. Cookies, Google Maps, visitor and ad measurement

We do not set any cookies of our own. We remember your choices about the map and measurement in your browser's storage (localStorage), which is necessary for this choice to work.

Google Maps, provided by Google Ireland Limited (Gordon House, Barrow Street, Dublin 4, Ireland), is loaded only with your consent (Art. 6(1)(a) GDPR and Section 109(8) of Slovak Act No. 452/2021 Coll. on Electronic Communications). When the map is shown, Google may set cookies and process your IP address, including in the United States. Google is certified under the EU-US Data Privacy Framework. For more information, see Google's Privacy Policy.

You can withdraw your consent at any time using the “Cookie settings” link at the bottom of the page.

Visitor and ad measurement (Google Analytics, Google Ads). If you consent, the Google tag from Google Ireland Limited is loaded on the website. It shows us how visitors use the website and how many people contacted us after clicking our Google ad (by clicking the phone number or sending the form), and Google may also use it to personalise ads. Google then sets cookies (e.g. _ga, _gcl_au) and processes your IP address and data about your visit, including in the USA (EU-US Data Privacy Framework). We do not send Google the name, phone number or message you enter in the form. The legal basis is your consent (Art. 6(1)(a) GDPR and § 109(8) of Slovak Act No. 452/2021 Coll.). You can withdraw your consent at any time via the “Cookie settings” link at the bottom of the page.

4. When you contact us

If you call us, send us an e-mail, use the contact form on our website or message us on Facebook or Instagram, we process your name, contact details and the content of your message so that we can reply and arrange an appointment. The legal basis is taking steps at your request before entering into a contract (Art. 6(1)(b) GDPR), or our legitimate interest in answering your questions (Art. 6(1)(f) GDPR). If you describe health problems in your message, we process this information only for the purpose of providing health care (Art. 9(2)(h) GDPR).

We keep this data while handling your request and then for a maximum of 1 year, unless it becomes part of your medical records. When you communicate with us via Facebook or Instagram, Meta Platforms Ireland Limited is a separate controller and its privacy policy also applies.

Contact form: we process the name, phone number and/or e-mail address and the message you enter in the form. The message is delivered by e-mail through the mail server of our hosting provider, WebSupport s.r.o., to our mailbox operated by Google Ireland Limited (Gmail). If the e-mail cannot be sent, the message is temporarily stored in a protected folder on the server outside the public part of the website and deleted once your request has been dealt with. Please do not include more health information in the form than is needed to arrange an appointment.

5. Patients of the practice

If you become our patient, we process your personal data, including health data, in your medical records for the purpose of providing health care, in accordance with Slovak Act No. 576/2004 Coll. on Health Care and related legislation (Art. 6(1)(c) and Art. 9(2)(h) GDPR). We keep medical records for the period required by law.

We share your data only where required by law, and with our partner dental laboratory to the extent necessary to make your dental restoration. If you apply to pay for your treatment in instalments, your data is processed by Quatro as a separate controller under its own terms.

6. Your rights

You have the right to access your personal data, to have it corrected or erased, to restrict its processing, to data portability, and to object to processing based on legitimate interest. Where processing is based on consent, you can withdraw it at any time; this does not affect the lawfulness of processing before withdrawal. Some rights (such as erasure) may be limited by our legal obligation to keep medical records.

To exercise your rights, e-mail us at storkdent@gmail.com. We will reply without undue delay and within one month at the latest.

You also have the right to lodge a complaint with the supervisory authority: Úrad na ochranu osobných údajov Slovenskej republiky (Office for Personal Data Protection of the Slovak Republic), Hraničná 12, 820 07 Bratislava 27, dataprotection.gov.sk.

7. Other information

We do not use automated decision-making or profiling. Your data may be transferred outside the European Union only in the cases described above (Google Maps, Google Analytics and Google Ads, Meta social networks). We may update this policy; the version published on this page always applies.
